What exactly is in a 45,000 miles service?

When I bought the car there was a 45,000 miles service done 1000~ miles ago.

The engine oil seemed a little dirty for oil with just 1000 miles (if they indeed changed it).

There are no details of what's done.
First, keep in mind that the "45,000-mile service" is supposed to be done after 45K miles or 3 years, whichever comes first. So yours should have been done a long time ago.

The service recommended for when a car reaches 45K miles or 3 years, whichever comes first, includes the following items:

- change oil and oil filter
- adjust valves
- replace (flush) coolant

However, the intervals on some of those items may change with other services. For example, the coolant should be changed after 45K miles or 3 years, and after that, every 30K miles or 2 years, whichever comes first. The coolant is normally flushed, and the water pump changed, when the timing belt is replaced, so thereafter, the coolant should be flushed every 30K miles or 2 years, whichever comes first.

When was the timing belt last changed on your '93?

Any service that you don't have proof that it's been done, assume it hasn't. Which means, on a '93, you really should be having the timing belt and water pump replaced (I would do the cooling system hoses too), all the fluids (oil, brake, coolant, transmission) and filters (oil, air, fuel), valve adjustment, etc. Except what's shown on that $1000 receipt.
