you simply tilt the plane of focus via a tilt shift lens. the plane of focus can be shifted in any of the X-Y-Z axis which is fully dependent upon what you wish to show/hide.
the reason why the images look miniature via tilt shift photos is due to the the depth of field showing ONLY the nsx in focus while composed in a medium/wide shot. normally medium wide shots are shot via slower aperatures and sharper lenses giving an image that is sharp all around.
having a wide shot with DOF is what gives the illusion that the items in the frame look minature since extreme DOF is normally associated with macro photography or photographer of of normal sized items/objects.
