DIY: Interior reupholstering part 1 - The gluing

Is it necessary to remove the door cards to remove and recover each of the insert side panels? I couldn't tell for sure as you had the door cards removed.

The door handle insert you remove first with a trim removal tool or a screwdriver wrapped in a towel per the service manual. The other 4 "cards" or inserts are held on with those metal tabs.
 
Yes you have to remove the door cards to remove the insert panels. IIRC there are some metal tabs you'll have to bend a bit to get them to come loose.

For the door panels, do you glue the foam to the panel and then the leather to the foam or is the the foam just a spacer and then you glue just the portion of leather that's folded over the back? What type of glue/cement is used that doesn't smell too strong once installed?

The foam is just a spacer and doesn't require glue. Only glue the material to the back side of the panel (do not wrap the foam around the edges other wise you'll never get the panels to fit). Clean off the old glue first with lacquer thinner. The best glue to use is Dap Weldwood Landau Top and Trim adhesive. Unfortunately it only comes in 1 gal containers, but it will never fail. It's available on Amazon. No need to spray it, just brush it on. let it sit per the instructions and then press the material into place with a nylon roller.
