No "carnage", everything is plug and play into the factory harness.

Yes, Bluetooth is the main source of audio, but I installed a "Y" cable to allow an "aux" cable to be plugged in as well, if need be.

how is the volume controlled? just on the phone?

With this setup the volume is only controlled threw the phone, which is a little louder than stock the head unit. The Bluetooth receiver allows you to change the song, answer calls, pause audio, and access Cortana, Siri and whatever android uses as well.
