I had something similar which I eventually found out it is tge starting relay located on the bottom of the glove box. Have you look at that relay? I had a few relay failure that cause problem.

Probably over rev cause it. I know a nsx locally that had at least 2 engine failures. One of the engine with a valve hitting the piston and the other failure was putting a hole on the block. That nsx ecu has no rev limiter.
